You may obtain a form to request access by using the contact information listed at the end of this Notice. There will be a charge for requested copies. If you request an alternative format, we will charge a cost-based fee for providing your health information in that format. Your e-mail must be verified. There are potential security risks when sending Personal Health Information thru e-mail.

Your Healthcare Provider: Pure Perio must agree to your request to not share your personal Health Information with any health plan, or insurance company, when an individual is paying for an item or service out of pocket and wishes not to have it reported.

Information Collected Upon Registration: If you choose to submit information on the new patient form you will be required to submit certain personally identifiable information to Company. This happens in a number of instances, such as when you complete our new patient form, referral form, or if you desire to receive other information. Personally identifiable information that we may collect in such instances may include your full name, email address, address, telephone number, birth date, gender, Medical history, Dental history, Insurance information, Medication, Allergies, Social Security number and other information that you decide to provide us.

Use of Contact Information: We will NOT use your personal information to provide marketing materials to you unless specifically desired. We respect your privacy and will not use your information or provide it to any third party for marketing purposes.
